The Southern Health and Social Care Trust has hosted a consultation event to allow service users and carers to feedback on their experience of the transition moving from Children’s to Adult Services.

The Adult Disability Transition Team was set up in September 2015. The team of Social Workers, Nurses, Physiotherapists, Occupational Therapists and Speech and Language Therapists, support young people with disabilities during their transition into adulthood. Each young person has their own case manager who works with them and their family to develop an individual care plan and put in place the services they need for example, direct payments/self-directed support and respite if required.

Young people in attendance were offered a Taster Session in ‘Fit 4 U 2’ which offers adults with learning disabilities a range of physical activity and leisure opportunities in local community settings. They were also given the opportunity to sample Music Therapy which is available from Everyday Harmony.

Representatives from the Trust’s Promoting Well Being Team shared information with Carers on ‘Take 5’ which ‘teaches five simple steps to keeping ourselves feeling good’ and also on opportunities available through ‘Riding for the Disabled’.

Micéal Crilly, Assistant Director Disability Services for the Southern Trust said that he welcomed the opportunity to engage with young people and their carers to help shape future service delivery. He also thanked the Promoting Health and Well Being Team who funded the event.

Mairead Casey, Patient/Client and 10,000 Voices Facilitator for the Trust attended the event and outlined to carers how they could complete the 10,000 Voices survey which will give them an opportunity to provide feedback on their experiences of the Adult Transition Service by ‘telling their story’. 

The 10,000 voices is a PHA initiative acknowledging that patient and client contribution is fundamental to ensuring that services are commissioned to deliver better outcomes for patients, their families and carers.
